As lawful consideration for being permitted to participate in the Committed; The Scholar Sports Source activities, camps, clinics, clinics, tournaments I agree that I will not make a claim against, sue, attach the property of or prosecute Committed; The Scholar Sports Source and their agents, employees, partners, sponsors, vendors, volunteers for damages for death, personal injury or property damage which I may sustain as a result of my participation in these sporting activities. This release is intended to discharge Committed; The Scholar Sports Source and their agents, employees, partners, sponsors, vendors, volunteers from and against any and all liability, including for negligent actions, arising out of or connected in anyway with my participation in the sporting activities, camps, clinics, clinics, tournaments except for liability that may arise out of the willful or wanton misconduct of Committed; The Scholar Sports Source and their agents, employees, partners, sponsors, vendors, volunteers. I FURTHER ADHERE AND UNDERSTAND THAT SPORTS CAN INVOLVE PHYSICAL CONTACT BETWEEN PLAYERS, THAT SERIOUS ACCIDENTS OCCASIONALLY OCCUR DURING SUCH SPORTING ACTIVITIES, AND THAT PARTICIPANTS IN SUCH SPORTING ACTIVITIES OCCASIONALLY SUSTAIN SERIOUS PERSONAL INJURIES (INCLUDING DEATH) AND/OR PROPERTY DAMAGE, AS A CONSEQUENCE THEREOF, KNOWING THE RISKS OF PARTICIPATION, NEVERTHELESS, I HEREBY AGREE TO ASSUME THOSE RISKS AND TO RELEASE AND HOLD HARMLESS COMMITTED; THE SCHOLAR SPORTS SOURCE AND THEIR AGENTS, EMPLOYEES, PARTNERS, SPONSORS, VENDORS, AND VOUNTEERS WHO (THROUGH NEGLGENCE OR CARELESSNESS) MIGHT OTHERWISE BE LIABLE TO ME (OR MY HEIRS OR ASSIGNS) FOR DAMAGES.
I attest that I am eighteen (18) years old or older, am physically fit and have no known medical conditions which prohibit participation in this sport. I agree to follow all laws, rules and guidelines regulating the conduct of Committed, The Scholar Sports Source.
I HAVE CAREFULLY READ THIS AGREEMENT AND FULLY UNDERSTAND ITS CONTENTS. I AM AWARE THAT THIS IS A RELEASE OF LIABILITY AND A CONTRACT BETWEEN MYSELF AND COMMITTED; THE SCHOLAR SPORTS SOURCE
AND THEIR AGENTS, SPONSORS AND EMPLOYEES, AND I HAVE SIGNED IT OF MY OWN FREE WILL. I also agree that AGENTS, EMPLOYEES, PARTNERS, SPONSORS,
VENDORS, AND VOUNTEERS may use my photograph for marketing purposes, public display, and for promotional purposes

ME (OR MY HEIRS OR ASSIGNS) FOR DAMAGES. I attest that I am under (18) years of age, am physically fit and have no known medical conditions which prohibit participation in this sport. I agree to follow all laws, rules and guidelines regulating the conduct of Committed; The Scholar Sports Source. I understand and agree that I am solely responsible for the mechanical and/or operating condition of any and all sporting equipment I use, and I agree to continuously inspect and maintain that equipment, even if I have obtained any of the equipment from Committed; The Scholar Sports Source, their agents, sponsors and/or employees. I HAVE CAREFULLY READ THIS AGREEMENT AND FULLY UNDERSTAND ITS CONTENTS. I AM AWARE THAT THIS IS A RELEASE OF LIABILITY AND A CONTRACT BETWEEN MYSELF AND COMMITTED; THE SCHOLAR SPORTS SOURCE AND THEIR AGENTS, EMPLOYEES, PARTNERS, SPONSORS, VENDORS, ND VOUNTEERS, AND I HAVE

VERIFICATION INFORMATION:
Registering to participate in one of our events, we may collect a photo of your drive’s license, passport, green car, or equivalent identity document solely in purpose to verify your identity. We will not use your information for any purpose other than verifying your identity and will immediately discard this information following completion of the verification process.
COMMUNICATIONS:
In the event you contact us directly, we may receive additional information on your behalf without limitation of your name, email address, mobile number, message contents, and any additional information sent to us on your behalf. Committed, Sports Scholar Source offers certain Services to its clients specifically geared to children. Committed, Sports Scholar Source does not knowingly solicit, collect, maintain, or use personal information from children under 13 years of age., other than certain limited information provided by. Child’s parent or authorized guardian such as information provided to sign participants up for camps, clinics, seminars, and tournaments.
For children under 13 years if age, parents will provide certain personal information about or for their child as information is not collected from the child directly. If a child under age 13 then participates in any ongoing activity that requires the collection, retention, and use of personal information in addition to what has already been provided or authorized by the parent, we will also contact the parent or authorized guardian notifying the parent of the personal information received from their child and seek consent to retain this information. We do not knowingly collect names and e-mail addresses or additional personal identifying information from children under age 13 for these types of activities without notifying the parent via e-mail and giving the parent the option to remove their child’s information from the applicable database for that activity.
